Step 2 Certificate of Immunization Status (CIS)

After completing the enrollment packet, prepare your child’s current Certificate of Immunization Status (CIS)

Washington State requires child care programs to keep a current Certificate of Immunization Status (CIS) on file for every enrolled child.

The easiest way to get your child’s CIS is through MyIR Mobile at wa.myirmobile.com. You can also request a copy from your child’s healthcare provider.

Once completed, please upload your child’s updated CIS directly through the Procare App.
